Cancellation Policy
If an appointment is missed or a cancellation occurs with less than 24 hours’ notice, there will be a charge of $40.00. This fee will be due the day of the rescheduled appointment.
Exceptions: Emergencies do arise and do not always allow for 24-hour notice. In the case of a personal emergency, there is no cancellation fee.
Payment
Payment is due at each session. Payment is accepted in the form of cheque, cash, debit, Visa, and MasterCard.
Insurance
Check with your insurance company for details regarding your coverage and maximums. You will be responsible for paying the fees upfront and submitting your receipts to your benefit provider for reimbursement. I can provide you with a letter or receipts, depending on the proof required by your insurance company or EAP provider. Reimbursement remains a matter between you and your benefit provider.
If you require me to verify the charges, I will do so with your consent. My disclosure will be limited to advising the insurance company of my credentials, as well as verifying the billing information, the service provided, and person served. At no time will any information regarding the nature of content of therapy be provided without your signed consent.
